What is the Fall Campaign?
Every year, Hoo’s Well offers employees and spouses covered by the UVA Health Plan the opportunity to participate in a Spring and Fall Challenge.
The Fall Campaign encourages participants to complete three steps toward their good health to earn monetary rewards. Complete a Biometric Screening or submit a Physician Screening Form to earn $200, complete an online health risk assessment to earn $100, and/or complete the Fitness Challenge to earn $200. Each step earns a different monetary reward and you can complete one or all three steps to earn!
Who is eligible to participate?
All UVA employees and spouses on the UVA Health Plan are eligible to earn $500 each for completing three steps toward good health. All rewards will be paid in the UVA Health Plan subscriber’s February 2018 paycheck. You must be on the UVA Health Plan and earning a paycheck at the time of payout. All rewards are considered taxable income.
Retirees, grant funded post-doctoral fellows, and COBRA enrollees are *not eligible for rewards.
I am a new employee at UVA. When am I eligible for Hoo’s Well rewards?
Employees must have elected their benefits by August 1, 2017 to be eligible for Fall Campaign rewards. If you are a new employee and you elected Health Plan benefits on or before August 1, 2017 you will be eligible to participate.
It can take 30 days from the date you submit your health benefits election for your information to be recognized in the ActiveHealth portal or to schedule a biometric screening. What’s in it for me?
Eligible employees who complete all three steps between August 1 and October 27, 2017 will receive $500 in their February 2018 paycheck (taxable income). Spouses enrolled on the UVA Health Plan
can also complete three steps to earn an additional $500 in the subscriber’s paycheck.
How do I keep track of my participation in the Fall Campaign?
To get started, go to the ActiveHealth Portal (www.myactivehealth.com/hooswell). Create an account if you have not already done so. If you have participated in a previous challenge, you will login with your current account. Spouses will need to create their own separate account to keep track of their individual activities.
Once you have logged in, click on “Earn My Rewards.” Locate the activity card you wish to complete and click “Work On It” then click “View/Track My Activity.”
What do I do if I am having trouble logging into the ActiveHealth portal?
Contact the ActiveHealth customer service number at 855.231.3715 for assistance.
Does my spouse need to create a separate account in order to participate?
Yes. Spouses covered by the UVA Health Plan will need to log in to www.myactivehealth.com/hooswell and create their own account in order to complete the three steps in the Fall Campaign.
Why am I not seeing my spouse’s information in my account?
In an effort to protect your privacy and offer a secure tracking portal, you will only be able to see your own personal information and progress in the ActiveHealth portal. In order for your spouse to participate in the Fall Campaign, he/she must create and account or sign in to www.myactivehealth.com/hooswell separate from your account. Our system will recognize him/her as your spouse when they submit their details. Encourage your eligible spouse to participate in the Fall Campaign to earn additional rewards!
Remember, all rewards earned by the spouse on the UVA Health Plan will be deposited in the subscriber’s paycheck in February 2018.
How long do I have to complete the challenge?
The Fall Campaign is open from August 1 through October 27, 2017. The deadline to complete all three steps is October 27, 2017. This allows all participants a 13-week period to complete these healthy actions.
Keep in mind you can only record one workout toward the Fitness Challenge per day. If you don’t start tracking your activity on or before Sept. 6, 2017, you will not have enough days left before the deadline to complete all 52 workouts required to earn $200 for this step!
What will happen to my health information that is collected from the Hoo’s Well fall campaign (i.e. biometric screening, online health assessment, and fall fitness challenge details)?
Hoo’s Well wants to provide you with the appropriate resources, so that you have the tools to be healthy. The information you’re submitting is voluntary and is being collected and submitted to HIPAA approved Health Plan Partners who the UVA Health Plan has signed a HIPAA Business Associate Agreement with to perform, permitted by HIPAA, for “treatment, payment and health care operations” purposes, including follow-up health-education, disease management, and improvement of the UVA Health Plan and its Hoo’s Well program. Our providers may use the information collected to send you invitations to participate in health improvement programs that are specific to your needs. The invitations to these programs are totally voluntary and kept confidential, between the providers and the UVA Health Plan. Although participation is encouraged, you are welcome to decline participation for any invitation that you may receive. The information is also used at an aggregate level, to report on the UVA Health Plan participants health status as a whole. All reporting at the institutional level is DE identified and in aggregate form. For example, we may report that 8% (799) of our participants, report tobacco use.
What if I used to work at UVA but now I am a dependent of someone who works at UVA or vice versa?

As a Basic Health high deductible health plan (HDHP) enrollee, can I elect to contribute my Hoo’s Well rewards to my Health Savings Account (HSA)?
Yes, if you are on the Basic Health plan and have a Health Savings Account (HSA), you can elect to have some of your wellness rewards placed in to your HSA. These rewards include the biometric screening, the online health assessment, the fall fitness challenge and the spring fitness challenge. Please complete the HSA One Time Request Change Form. Follow the instructions on the form and submit by the deadline, January 15, 2018.
Fitness Challenge FAQ
Do I have to use a Fitbit, App, or Activity Tracker to participate in the Fall Fitness Challenge?
No. You do not need to use an activity tracker or mobile application in order to participate in
the challenge. This feature is available to make tracking and logging your activity easier. If you’d prefer to manually enter your activity by self-reporting it, you will still earn credit by doing so.
I thought I connected a fitness tracker last year, but my steps don’t seem to be tracking. What should I do?
Click on “Earn Your Rewards” in the ActiveHealth Portal (www.myactivehealth.com/hooswell). Under the “Fall Fitness Challenge” click on “Work On It”. Under the drop-down menu, you will see “Click HERE to sync your compatible fitness device or app.” Follow the link and select the prompts to connect the app or device you wish to sync with the portal. If it shows your device is currently connected, but no activity information is tracking, you can reboot your device in the portal by clicking “Delete”. Don’t worry, this won’t delete your information in the portal or on your tracking device. It simply allows you to disconnect and reconnect the device to ensure your tracker and the portal are communicating with each other.
What if I want to use an activity tracker, but it is not compatible with the ActiveHealthPortal?
If you have an activity tracker you’d prefer to use, but it is not compatible with the ActiveHealth
portal, you may still use this to track your activity or steps. Be sure to keep track of your step
count each day, and then manually log this activity from your device into the portal. A list of
compatible devices can be found at www.myactivehealth.com/hooswell. Under “Earn Your
Rewards” you will find a “List of Approved Fitness Devices”.
Will an Apple Watch sync with the ActiveHealth Portal?
Apple products do not sync on the ActiveHealth Portal. However, some data from certain apps may sync. For example: If you track an activity using the Runkeeper app from the Apple watch, the data should be sent to the ActiveHealth Portal. Please confirm that your data is syncing to the Portal if you choose to use the Apple Watch.
Will we receive free Fitbits again?
In January, 2016 Hoo’s Well gave free Fitbits to anyone on the UVA Health Plan. We are not able to give out free Fitbits at this time but we can direct you to the UVA Bookstore to purchase one for yourself. The UVA Bookstore/Cavalier Computers offers a 10% discount. Please stay tuned for future promotions.
Can I log both time and steps to get credit?
Yes. You can log a mixture of exercise time and steps to earn credit toward completing the
challenge. Be sure that the activity or steps logged reach the minimum requirement per day
(30+ minutes or 7,000+ steps) in order for them to count toward your goal of 52 workouts.
Why have the steps increased from 6,000 to 7,000?
The American Heart Association recommends that the average adult achieve 10,000 steps each day. We are still falling shy of this goal, and based on the amount of time in the challenge and the required 52 workouts, our employees should have plenty of time to complete this if they choose to participate. The Spring Fitness Challenge encouraged employees to reach 7,000 steps and we want to help you maintain this level of activity. You can earn $200 for completing the 52 workouts AND spouses covered by the Health Plan can also earn $200 as well. We do not plan to increase the step count in the near future. We will stay at 7,000 steps for next year’s Fitness Challenges and also keep the option of recording time vs. steps if that option is easier for people to reach the goal. We want to make the Challenge fun and are mindful of busy schedules, but still want to encourage folks to increase their daily activity and challenge themselves to get more steps in their day!
What if I feel I cannot complete the activity due to illness or a physical condition?
If you have a medical condition that makes you unable to participate, or your physician has indicated it is medically inadvisable for you to attempt the challenge, either you or a physician may contact us at 434.982.0123 or email@example.com to discuss the reasonable alternative.
In order to process your reasonable alternative within a timely fashion, you are encouraged to
reach out to Hoo’s Well to make your request by Sept. 29, 2017. Please note: A reasonable alternative will be considered incomplete if you do not contact Hoo’s Well to request by noon of the challenge end date (10/27/17).
What resources are available to help me complete the Fitness Challenge?
Hoo’s Well offers many fitness, stretching, and walking opportunities to help you meet your activity goals. Our Fall Schedule is available online and always updated to allow you to find a class that works for your schedule and personal interests. Visit the main homepage of the Hoo’s Well website (look under “Resources” or on the left navigation bar for “Schedule”) for a full calendar of activities and FREE drop-in classes at IM-Rec this fall. Options include (but are not limited to) swim, Karate, tennis, yoga, weight training, Pilates, and more!
If I am on the Basic Health Plan, will my rewards be deposited in my HSA?
No, your reward will be deposited in your February, 2018 paycheck.
Biometric Screening FAQ
How do I sign up for a biometric screening?
Sign up here or call 1-877-366-7483 to register for your on Grounds biometric screening. You will need to create a new account. Your spouse will also need a new account. If you have any trouble with this, call their number at 1-877-366-7483 to schedule your appointment.
I did my biometric screening but the webpage is saying I have not completed it. What should I do?
It can take up to 21 days from the date you completed your biometric screening until it shows as completed in the ActiveHealth portal. An email will go out in October to remind you to check that you have completed all steps of the Fall Campaign in the ActiveHealth portal. Please log in to ensure all activities have been completed to earn your reward. There will be an appeals process if needed.
What is the last day I can do a biometric screening on Grounds?
Biometric screenings will be held on Grounds from August 1- October 27th. Please sign up here to ensure you can get an appointment. Space fills quickly! If you are not able to book a biometric screening on Grounds, you can submit a Physician Screening Form and attach your lab results to earn credit. You can use lab results from August 1, 2016, through October 27, 2017 instead of attending a screening. Print the physician screening form. Follow the instructions on the form to submit on or before October 27, 2017, to earn your reward.
What type of appointment should I schedule if I choose to see my physician and submit a Physician Screening Form rather than attending a biometric screening on Grounds?
Please schedule your appointment as an Annual Physical rather than a biometric screening. Preventive Annual Physicals are covered at 100%. If you request a biometric screening, you may incur a charge.
Can my doctor submit my biometric screening information?
No, the employee/spouse is responsible for submitting their own physician screening form and attaching your doctor’s office’s copy of your lab results. You can either email, fax, or mail this information to:

I am having trouble faxing my Physician Screening Form, what should I do?
Try scanning and emailing the form to firstname.lastname@example.org or, if you’re still having trouble, call 1.800.658.5821.
Will you accept lab results from last year?
You can submit a Physician Screening Form and attach your lab results to earn credit. You may use lab results from August 1, 2016, through October 27, 2017. Print the physician screening form. Follow the instructions on the form to submit on or before October 27, 2017, to earn your reward.
How long will it take for my biometric screening to show as completed in the Active Health Portal?
Please allow up to 21 days for your biometric screening to show as completed in the ActiveHealth portal. If you do not see this step completed in the portal, please contact email@example.com or AskHR at 434-982-0123.
Online Health Assessment FAQ
How do I complete my online health assessment?
Login to www.myactivehealth.com/hooswell and create an account if you have not already done so. Your spouse will need his or her own account as well. Click on “Earn Rewards”.
Can I access and print my ActiveHealth personal health report once I’ve completed it?
You can access/print your personal health report by clicking on “Earn Your Rewards” and then selecting “Past Rewards” from the drop-down menu. If you wish to access your past Health Risk Assessment answers, click “Dashboard” then “Health Assessment”. Scroll down and you will see “Compare Answers” which will allow you to see your “past record” and “current record” of health history.
